Our Mission
The mission of the Neighborhood Care Coordination Project is to assist those living with HIV/AIDS with the coordination of a wide variety of health and social services in their own neighborhood. It respects cultural diversity, emphasizes confidentiality, and strives to ensure the client’s freedom of choice and self-determination. Its comprehensive services are rendered in a safe, secure, and non-judgmental environment.
What We Do
- Orient client to services
- Complete an initial interview and assessment with the client
- Implement and coordinate services
- Develop an individualized care plan with the client according to the client’s need and available services
- Monitor client’s progress and evaluate the individualized care plan
- Link the client with various resources
- Ensure people living within the service area are aware of the availability of the program through collaborations, educational presentations, and general recruitment
- Advocate for and with the client
